Job Summary
As a Case Manager at Oceanrise Healthcare Agency Ltd, you will play a critical role in coordinating care for our clients. You will be responsible for assessing client needs, developing individualized care plans, and ensuring that all services are delivered effectively and efficiently. Your expertise will help bridge the gap between clients and healthcare providers, ensuring that each individual receives the highest standard of care.

Key Responsibilities

  • Conduct comprehensive assessments of clients’ physical, emotional, and social needs.
  • Develop and implement tailored care plans in collaboration with clients, families, and multidisciplinary teams.
  • Monitor client progress and adjust care plans as necessary to meet evolving needs.
  • Serve as the primary point of contact for clients and their families, providing guidance and support throughout their care journey.
  • Collaborate with healthcare professionals including doctors, nurses, social workers, and therapists to coordinate services.
  • Maintain accurate documentation of client interactions, assessments, and progress notes in compliance with regulatory standards.
  • Advocate for clients’ rights and ensure they have access to necessary resources and services.
  • Participate in case conferences and team meetings to discuss client cases and share insights.

Qualifications

  • Bachelor’s degree in Nursing, Social Work, Psychology or related field; Master’s degree preferred.
  • Relevant case management certification (e.g., CCMC or ACM) is highly desirable.
  • Minimum of 3 years’ experience in case management or a related healthcare field.
  • Strong understanding of healthcare systems, community resources, and patient advocacy.
  • Excellent communication skills with the ability to build rapport with diverse populations.
  • Proficient in using electronic health records (EHR) systems.
